What will be done with my information?

A letter will be provided on your behalf to your physician, agency, or any organization requiring documentation of your counseling. As part of your intake process, you will complete a release of information form for each entity requesting a letter to ensure proper communication and confidentiality.

How much will this cost?

Each required session is billed at a rate of $200 per session, which includes both the time spent in your counseling session and the preparation of the letter provided to your physician, agency, or other requesting entity.

When will this process be complete?

The timeline for completing this process depends on how quickly we are able to coordinate and link everyone’s schedules. The letter will be prepared and sent by the end of the week in which the final session is conducted.
